Sample instruments that have been used in studies to test the Health Promotion Model are presented here. These instruments focus on physical activity, but can serve as exemplars for the development of instruments focused on other health behaviors. A measure of regulatory self-efficacy, efficacy for overcoming barriers to physical activity, is presented here. This instrument has been used with adolescents. Scoring instructions are provided.en_US
