Methodology
  • All patients discharged from Michigan Medicine between 2016 and 2021 with a diagnosis of acute STEMI were identified. For each patient, a single digital 12-lead EKG >1 week pre-STEMI and >1 week post-STEMI were collected. Associated demographics and clinical outcomes, including mortality and heart failure, were also collected.
Description
  • It is difficult to model outcomes in patients post-ST Elevation Myocardial Infarction (STEMI), particularly onset of heart failure. The acute insult following a myocardial infarction and chronic degeneration seen in HF involve a similar process where a loss of cardiomyocytes and abnormal remodeling lead to pump failure. This process may alter the strength and direction of the heart’s net depolarization signal. The investigators hypothesized that changes over time in unique parameters extracted using vectorcardiography (VCG) have the potential to predict clinical outcomes in patients post-STEMI and could eventually be used as a non-invasive and cost-effective surveillance tool for characterizing the severity and progression of HF to guide evidence-based therapies.

1. Pre_Post_STEMI_ECGs.zip: This folder contains two CSV files each for a subset of STEMI events evaluated in the study. For each numbered STEMI event, there is one CSV file with data pertaining to the pre-STEMI ECG, and one CSV file with data pertaining to the post-STEMI ECG.

2. STEMI ECG Metadata.xlsx: This file contains metadata pertaining to the STEMI events represented in the zip folder. This data, which is described in additional detail below, describes patient characteristics for each STEMI event.

- STEMI No.: Indicates which STEMI event is being referenced
- Final Study Inclusion: Only 162 of the 266 STEMI events represented here were selected for inclusion in the study's final analysis. Y indicates inclusion, N indicates exclusion.
- STEMI Age Indicates patient's age in years at the time of the STEMI
- Gender: Indicates if the patient is male or female
- Diabetes: Indicates if patient had a history of diabetes
- Hypertension: Indicates if patient had a history of hypertension
- Tobacco Use: Indicates if patient had a history of tobacco use
- Cerebrovascular Disease: Indicates if patient had a history of cerebrovascular disease
- Pre-STEMI ECG: Number of days by which pre-STEMI ECG preceded STEMI event
- Post-STEMI ECG: Number of days by which post-STEMI ECG followed STEMI event
- HF Onset: Indicates number of days between heart failure diagnosis and STEMI event. Negative values indicate a diagnosis of heart failure prior to STEMI event, while positive values indicate a diagnosis of heart failure following STEMI event
- Mortality: Indicates number of days between STEMI and all-cause mortality
