reader = central reader identifier (de-identified)

SEMA_EMR_colon = severity of colon
	0 = Endoscopic remission
	1 = Minimal disease
	2 = Mild disease
	3 = moderate disease
	4 = severe disease

SEMA_EMR_ileum = severity of ileum
	0 = Endoscopic remission
	1 = Minimal disease
	2 = Mild disease
	3 = moderate disease
	4 = severe diseaseSEMA_EMR_segments = number of colonic segments involved

SEMA_EMR_segments
	Number of colonic segments involved

SEMA_EMR_score = total SEMA-CD score from colonoscopy report
	Calculated as SEMA_EMR_ileum + (SEMA_EMR_colon * SEMA_EMR_segments)

image = flag for whether or not colonoscopy report contained images
	0 = No colonoscopy images included
	1 = Colonoscopy image(s) included
