dc.description.abstractThe authors describe an innovative approach to increasing COVID-19 vaccination uptake by a primary care provider-led targeted outreach protocol using zip codes, EHR data and phone canvassing. Preliminary findings show that primary care has a critical role in increasing access, equity, and patient uptake of COVID-19 vaccines.en_US
